Why Loose Vagina is caused after childbirth?

If we consider that the vagina before childbirth is usually 2-2.5 cm in diameter and a baby's head will pass through there, which is about 10 cm, then we realise that even if the vagina returns to its former diameter, it will certainly never return to 2 cm. So after 1 or 2 normal deliveries, a relaxation in the female vagina is usually observed. Quite often you ask us if this vaginal relaxation is observed, if the woman gives birth by caesarean section. The answer is no! But where exactly does vaginal relaxation cause problems? In many things in a woman's life, as you will see below.

What problems does Loose Vagina cause?

First of all, a loose vagina is more "open" to the entry of microorganisms and is therefore more vulnerable to vaginal infections. Thus, it is not uncommon for women with loose vagina to suffer very frequently from vaginal infections. At the same time, urinary incontinence can occur after childbirth, even in very young
women. Small leakage of urine when laughing or coughing is most likely due to relaxation of the pelvic floor of the vagina after childbirth, which worsens over time. Also another problem, in women with loose vagina after childbirth, is that the reduced sexual satisfaction they have after childbirth and thus their psychology and self-confidence are significantly affected as well.

Causes of Loose Vagina, other than Childbirth

Ageing
As women age, their bodies undergo significant changes, including a normal decrease in collagen and elastin production, especially after age 50. These changes can lead to decreased firmness in the vagina.
Hormonal changes
Hormonal fluctuations that occur during menopause or perimenopause, with the significant decrease in oestrogen production, lead to loose vagina, vaginal dryness, thinning of the vaginal walls and vaginal atrophy.
Genetics

Vaginal dryness is a fairly common condition in menopausal women due to the reduction in oestrogen levels. Oestrogens are responsible for the continuous production of collagen and elastin that keeps the vaginal walls healthy, elastic and firm and on the other hand for the normal secretions of the vagina, which protect it from irritations and pathogenic microorganisms, as they act as a natural lubricant, making also the sexual intercourse more comfortable.

That's why, during menopause, when oestrogen levels drop dramatically, most women start to feel the annoying symptoms of vaginal dryness. That's why in addition to menopausal women, sometimes younger women may
also experience symptoms of vaginal dryness. The causes may be the washing of the sensitive area with inappropriate products, various conditions such as Sjogren's syndrome, surgical removal of the Bartholin's glands, surgical removal of the ovaries, taking drugs (mainly antidepressants and contraceptives), allergies and frequent vaginal infections.

Women with vaginal dryness begin to notice some differences in the sensation of the vagina, both during urination and during intercourse with difficulty and pain during sex, a condition also known as "dyspareunia".

Treatment of vaginal dryness has always been with topical oestrogen gels and creams, which were used daily or just before intercourse. However, success rates and compliance were low, as pain and dryness returned immediately after the treatment was discontinued. That has increased the need for new, more effective forms of treatment for this specific problem.

Nowadays the Vaginal Laser has replaced all the ineffective forms of treatment used in the past and may require 1-3 treatments, performed in the Clinic, completely painless and at low cost. It is the fastest, safest and most effective treatment for vaginal dryness and is already the treatment of choice for vaginal problems during menopause in many European countries.

As we mentioned, vaginal atrophy and dryness are caused by a drop in oestrogen levels, as oestrogen keeps the vaginal mucosa healthy, hydrated and elastic. This happens mainly in menopausal women, but sometimes the discomfort starts years before (premenopause) and in others it appears several years after its onset.

A drop in oestrogen is also observed during breastfeeding, removal of the ovaries and during treatment of breast or ovarian cancer (chemotherapy, radiation therapy, hormone therapy). When oestrogen levels are low, vaginal tissue becomes thin, dry and atrophic. Symptoms can be mild to severe and these are: vaginal dryness, vaginal itching and burning sensation, vaginal irritation and pain during sexual intercourse, frequent microbleeds and vaginal infections.

Urine leakage can occur for various reasons, but the most common cause is the relaxation of the pelvic floor and the vaginal wall. The most common form is stress urinary incontinence and 3 out of 10 women experience this incontinence (stress urinary incontinence) in their daily activities, when they cough or sneeze or even when they laugh.

Urinary incontinence in women is not exclusively related to ageing as many believe, but also to the relaxation of the vaginal walls which can no longer support the overlying wall of the bladder - urethra and this makes a woman's daily life difficult on multiple levels. Urinary incontinence is diagnosed with a urodynamic test, which assesses the extent of urine loss and identifies the type of incontinence, although the most common type is stress urinary incontinence. Until now, for urinary incontinence, surgery has been the only option for these women (even in cases of minor or moderate incontinence).

This surgery is done through a small incision in the vagina to place a supportive polypropylene tape under the urethra to strengthen it and that helps with the urinary incontinence. Now 7 out of 10 cases of urinary incontinence can be treated with the Vaginal Laser, which is an ideal treatment for mild to moderate incontinence. Its comparative advantage over surgery is that it is bloodless and more economical. It works by causing an increase in collagen and elastin and an increase in the tightening of the area.

If you use oestrogen creams for topical application inside the vagina, you should not use them for the last 5 - 7 days before your treatment.
