What are Spider Veins?

These are very small and thin (0.1 - 1 mm in diameter), broken or dilated capillary blood vessels under the skin, which usually appear on fair skin having red, purple or blue colour. Spider veins aren’t actually veins. They’re damaged capillaries, venules and arterioles. These are all small blood vessels that connect with larger blood vessels as part of the circulatory system. In the majority, they first appear on the area around the ankles and gradually make their appearance upwards on the calves and thighs. 

What causes Spider Veins on Face?

Spider veins on the legs aren’t actually veins as we said and are commonly a sign of stage 1 venous insufficiency. They are small blood vessels that connect with larger veins as part of the circulatory system. As you know, veins return blood from the legs to the heart. Inside the veins there are specific valves that prevent the reflux of the blood and if these valves start not to function properly, the venous pressure of the lower extremities is increased, due to the reflux of blood backwards.

The insufficiency of the valves and the reflux of blood, increases the venous pressure of the lower extremities which in its turn is opening new paths in the venous network, which are the small superficial capillaries that are called spider veins.

Ageing

As the skin becomes more atrophic and thinner and loses its elasticity with age, due to a decrease in collagen and elastin, the superficial capillaries break more easily and this is how spider veins appear.

Exposure to the sun

Prolonged exposure to the sun can cause skin atrophy and damage to blood vessels, thinning their walls and making them more prone to breaking.

Alcohol

Daily and heavy consumption of alcohol can cause blood vessels to dilate and consequently make them more visible under the skin. 

Heredity

Some people have a hereditary history of spider and varicose veins and if their parents had them, then are more likely to develop the same.

Are Spider Veins on legs the same as Varicose Veins?

No, spider veins are not the same as varicose veins. Spider veins are the first stage of venous insufficiency as we said above, while varicose veins are the second stage of venous insufficiency. Spider veins are the very thin capillaries, while varicose veins are larger diameter, dilated veins that run across the surface of the legs.

Spider veins are more of an aesthetic issue, while varicose veins, in addition to being an aesthetic issue, also have clinical symptoms for patients with a feeling of heaviness and burning in their legs, especially at night. For the diagnosis of varicose veins, it is necessary to perform a vascular triplex of the lower extremities followed by their surgical removal by a Vascular surgeon, while for spider veins, the diagnosis is clinical and their treatment is non-invasive, either with vascular laser or with sclerotherapy.

Sclerotherapy is done by injecting a hardening substance into the lumen of vessels (which should have diameter of 3mm or more) and usually the medicinal substance polidocanol (Aethoxysklerol) is used alone or mixed with a certain amount of air to create foam.
